Removes the BROAD_SCOPE_FLAG_VERSION/decideBroadScopeFlag pipeline,
the triage.ts call site that wrote `broadScopeFlag` source metadata
and emitted task:broad-scope-flagged-at-triage audit events, the
DatabaseMutationType enum entry, the diagnostic doc section, and the
associated unit + reliability-interaction tests.

87da1cb757 test(dashboard): align github-tracking-documentation phrases with current docs 
The doc was rewritten to cover "every task-creation path" (with an
explicit enumeration of surfaces) instead of the older "task creation
flows (including ...)" wording, and uses "best-effort and non-blocking"

b08a57f325 feat(FN-3931): recover triage planning model session keys in triage engine 
The merge fixes triage planning model session key recovery in the engine, enabling the triage agent to properly use default model keys when resuming planning sessions. Tests were added to `triage.test.ts` to assert the correct behavior.
