Adds health-surfacing logic for timer-triggered stale heartbeats in the engine, a new `agentHealth.tsx` dashboard utility with tests, and documentation updates for the repair behavior.

This merge restores the canonical agent lifecycle with termination scoped at the run level (FN-3580, 4 steps), adds sender-side wake recipient override for messages, and introduces test isolation CI enforcement with a stuck-requeue race fix. UI changes remove terminated-agent indicators from AgentDe

This merge completes the agent terminated state alignment (FN-3535), adding "running → terminated" transition support with consistent styling and lifecycle controls across the heartbeat engine, agent store, and dashboard UI, plus plugin author documentation improvements (FN-3537) and plugin loader t

Drops "terminated" from AGENT_STATES. The agent lifecycle now runs through
idle | active | running | paused | error. paused (carrying a pauseReason)
absorbs every former terminated use case — manual stop, heartbeat run
termination, spawned-child cleanup. Run status (agentRuns.status) is
unchanged: "terminated" stays a valid run-status value.
AGENT_VALID_TRANSITIONS allows direct any→idle transitions so resetAgent
no longer needs the intermediate hop.
Stack-wide:
- core/agent-store: lastError clearing + resetAgent simplified.
- engine/agent-heartbeat, executor, in-process-runtime: terminated state
writes → paused; halt-state listener fires on paused/error.
- dashboard: AgentsView/AgentListModal/AgentDetailView lose the Terminated
badge/option/state-block; agent pickers no longer filter terminated;
agentHealth drops the Terminated branch; routes/state cast widened to
the new AgentState union.
Tests across core and engine updated to assert paused for AgentState and
left "terminated" intact for run-status assertions.

Enriched agent heartbeat callbacks with a `reason` parameter propagated through the engine, in-process runtime, and dashboard components (AgentDetailView, AgentListModal, AgentsView). Added a new `agentHealth.tsx` utility and corresponding test file to surface the reason in UI health status, with te

- Resize the dashboard TUI splash logo to scale with the terminal: a Colossal-font variant kicks in on terminals ≥70 cols × 16 rows; gradient palette is now strictly white → blueBright → blue (no cyan/magenta)
- Lower-clip TUI Kanban cards to a single-line title and constrain each column with overflow hidden so cards no longer bleed past the header row
- Center the TerminalModal vertically by overriding overlay alignment via :has() and trimming the modal height
- Match GitHub Import modal width to the file browser modal (90vw / 1600px max)
- Render the Usage indicator as a top-right popover on desktop instead of a centered modal (mobile keeps the full-screen sheet)
- Widen SetupWizard / Model Onboarding modals (540→880px, 560→880px) and replace the washed-out done step indicator background with a solid success swatch
- Drop the duplicate mailbox icon button from the desktop header (view switch covers it; overflow menu still surfaces it on compact)
- Loosen agent health detection: bump the heartbeat grace multiplier 2× → 4× and the staleness floor 60s → 5min so transient ticks don't flag agents Unresponsive

agentHealth.tsx:
- Drop the "Disabled" label and isHeartbeatEnabled helper. Server-side
heartbeat scheduling is driven by agent.state (active/running = armed),
not by the legacy runtimeConfig.enabled flag, so surfacing "Disabled" for
agents with a stale flag on disk was misleading.
- Replace the elapsed > heartbeatTimeoutMs check with elapsed > 2× effective
interval (with a 60s floor). heartbeatTimeoutMs is the per-run work budget,
not a between-tick freshness window; using it made any agent configured
with a multi-minute+ interval show Unresponsive the moment it got more
than 60s old. The effective interval resolver now falls back to the
server-side 1h default, so agents never configured via the dropdown and
agents with an explicit interval get consistent treatment, differing only
by scheduled cadence.
AgentDetailView.tsx:
- Remove Stop buttons from active/paused/running/error (mirrors the earlier
AgentsView cleanup — Pause/Resume/Retry cover reversible transitions,
Delete stays on idle/terminated for teardown).
- Add a "Next Heartbeat" info row alongside "Last Heartbeat" for ticking
agents (active/running), computed as lastHeartbeatAt + effective interval.
Tests rewritten to cover the new semantics; dropped 14 obsolete cases that
were pinned to the old heartbeatTimeoutMs-based check and the "Disabled"
branch. 42 agentHealth tests pass.

- Update getAgentHealthStatus to set stateDerived consistently across terminated, error, paused, running, heartbeat, idle, and disabled paths
- Update AgentDetailView, AgentListModal, and AgentsView to render icon-only health badges when labels are state-derived while preserving full labels via title tooltips
- Expand agentHealth tests with explicit stateDerived assertions and a table-driven semantics suite for representative states
- Apply a default 30s heartbeat interval when scheduler config is missing or invalid
- Dynamically register and update heartbeat triggers from AgentStore create/update events
- Register all non-disabled agents at startup and guard listener cleanup when stopping runtime
- Treat non-periodic agents as healthy in dashboard health checks to avoid stale-time false alarms
