fetchAgentLogsWithMeta was calling fetch(url) directly instead of
fetch(buildApiUrl(url)), so requests hit the SPA fallback instead of
the /api/tasks/:id/logs endpoint, returning HTML that silently failed
JSON parsing and resulted in empty agent log entries.

The task executor now sends tasks back to in-progress (instead of 'in-review'
with 'failed' status) when workflow step verification fails and retries are
exhausted. This allows the executor to attempt to fix the issues on the

in project-engine.ts.
