gsxdsm 0e8d743064 FN-8867: prevent false task wedge notifications

Prevent stale terminal snapshots and pause markers from notifying for progressing tasks.

- Reclassify the live task immediately before claiming wedge episodes.
- Require actual pause state for pause-derived wedge reasons.
- Cover resumed-task races and stale pause markers with notification tests.
- Document the wedge notification behavior and add a patch changeset.
