- Rename package from @gsxdsm/kb to @gsxdsm/fusion in package.json and metadata
- Update all documentation references (README, RELEASING, STANDALONE, AGENTS.md)
- Update root workspace scripts for new package name
- Update changeset files to reference @gsxdsm/fusion and @fusion/dashboard
- Update CHANGELOG.md with new package name references

Move file browser from task detail to top-level toolbar

Files can now be browsed and edited from the main dashboard toolbar
File browser shows the project root filesystem instead of task worktrees
Removes Files tab from task detail modal (no longer needed)
