gsxdsm e43dffad52 FN-8434: preserve evolving running plans

Keep Planning Mode's running plan as an evolving work product alongside every interview question.

- Carry model-authored running-plan updates through question responses without ending the interview
- Merge partial plan updates with prior title, deliverables, dependencies, size, and priority
- Derive safe fallback summaries without turning interview questions into deliverables
- Cover running-plan persistence and document the contract
