fix: refineTask/duplicateTask fail in backend (PostgreSQL) mode (#2253)
## Summary Eliminates the remaining backend/PostgreSQL-mode sync-SQLite (`store.db`) call sites — both the crashing ones and the try/catch-masked ones that silently degraded features. Found via a full audit of `store.db`/`archiveDb` residue after the PG cutover's per-site routing missed them. **Crashes fixed:** 1. **refineTask / duplicateTask** threw `TaskStore.db: SQLite Database is not available in backend mode`. Both create rows through `createTaskWithId` callbacks calling `store.atomicCreateTaskJson()` directly, bypassing `_createTaskInternal`'s backend routing. The shared helper now routes itself (soft-delete conflict check + non-destructive insert in one AsyncDataLayer transaction). 2. **Merger verification cache**: `getVerificationCacheHit` ran sync SQLite unguarded *outside* any try/catch in `runDeterministicVerification`; `recordVerificationCachePass` was swallowed so the cache never warmed. Both are now async with a PG branch. **Silent degradations fixed (features that were dead on PG):** - Workflow run-branch + foreach step-instance persistence (`saveWorkflowRunBranch`, `loadWorkflowRunBranches`, `clearWorkflowRunBranches`, `saveWorkflowRunStepInstance`, `loadWorkflowRunStepInstances`, `clearWorkflowRunStepInstances`) — executor crash-resume checkpoints were silently never persisted. - `getBranchProgressByTask` — returned an empty map, dropping `branchProgress` from task payloads. - `runPluginColumnTransitionHooks` — plugin `onEnter`/`onExit` column-transition hooks never fired (marker bookkeeping + non-locking task read now async). - `getTaskColumns` — dashboard treated all agent-linked tasks as non-terminal. - `getWorkflowStep` / `listWorkflowSteps` — stored workflow-step rows now read from `project.workflow_steps` (listing previously returned plugin steps only); `getLegacyWorkflowStepSnapshot` returns `undefined` on PG (legacy snapshot exists only in pre-migration SQLite). - `readRawProjectSettings` / `listWorkflowPromptOverridesForProject` — now read via the async layer. Import picker now surfaces all open issues instead of a silent first-30 slice, and reliably closes linked/imported issues when their task reaches Done. Import: - GitHubImportModal fetches up to 300 open issues in one request and pages the result client-side at 30/page with Previous/Next controls, a page indicator, and a truncation notice past the cap. - NewTaskModal reference picker limit 30 -> 100. - GitHubClient.listIssues pages the REST path (per_page loop until limit/exhaustion; PR-filtering no longer stops paging early) and lifts the gh path's 100 cap (gh --limit paginates internally); gh-CLI label filtering fetches the full cap before client-side OR filtering. Close-on-Done: - GitHubTrackingReconciler.runSweep isolates its three reconcile passes so a throw in one (e.g. the PG-backend deleted/archived pass) no longer starves the done-task + source-issue passes. Previously all three shared one try/catch with a silent swallow, so a single early throw disabled the whole close-on-Done reconcile backstop on every sweep, leaving linked issues open. Failures are now logged instead of hidden. Tests: REST multi-page paging, PR-filter-doesn't-stop-paging, Prev/Next UI, one-page (no controls), and per-pass isolation invariants. fix(FN-8004): make AI merge rejections actionable and stranded merges retryable (#2160)
Two follow-ups to FN-8004. Both were found by watching FN-8004's *own* merge livelock for 40 minutes — it turned out to be blocked by the very class of bug it was filed to fix. ## 1. AI merge rejections lost their reasons The reviewer prompt said **both** of these: > "**End with a single decision line**: `REVIEW_VERDICT: approve|reject`" > "**Then list each concrete reason as a bullet.**" Those are impossible to satisfy at once. Reviewers obeyed "End with" and wrote their reasoning *above* the verdict — but `extractRejectReasons` only scanned lines *after* it. So every such rejection collapsed to the placeholder `reviewer rejected the merge without a stated reason`, and that placeholder was then handed to the corrective re-merge pass **as its instruction**. The pass got no actionable feedback and just re-rolled the merge. An orphaned merge-active stamp was un-retryable by hand The Retry gate refused **every** merge-active status (`Task is not in a retryable state (current status: landing)`), while self-healing cleared stale stamps automatically minutes later. So a merger killed mid-flight — crash, engine restart, operator SIGTERM — blocked the operator's own escape hatch at exactly the moment they'd reach for it. FN-8004 hit this: a killed merge left `landing` stamped and Retry 400'd for the full sweep delay. `isStaleMergeActiveStatus` now lives in the leaf `merge-active-status.ts`, shared by `recoverStaleMergingStatus` and the Retry gate — so **the manual path can never be stricter than the automatic one**. This is the same one-concept-two-definitions bug as FN-8004's transient classifier, which is why it's worth fixing structurally rather than adding another special case. A live merge stays protected by two independent signals: it holds the in-process lease **and** refreshes `updatedAt` each phase. Staleness fails closed on an unparseable timestamp.
