Replace "break into subtasks" checkbox with explicit "Plan" and "Subtask" buttons

Task creation UI now shows two explicit buttons instead of a checkbox:

• "Plan" — Opens AI planning mode to refine the task before creation
• "Subtask" — Opens subtask breakdown dialog (feature coming in next release)
